8. CARCINOM ADENOID CHISTIC DE GLANDA LACRIMALA EXULCERAT - PREZENTARE DE CAZ -

Rezumat
Autorii prezinta cazul unui pacient de 63 de ani, internat la Clinica de Neurochirurgie Cluj Napoca, prezentānd carcinom adenoid chistic exulcerat al glandei lacrimale drepte. Tumora este extinsa medial, īn portiunea superolaterala a orbitei poste-rioare si lateral īn fosa ternporala. Clinic pacientul prezinta exoftalmie unilaterala dreapta de un an si jumatate. Tumora este exulcerata īn regiunea temporala dreapta de un an. Examenul CT-scan evidentiaza formaxiunea tumorala izodensa cu localizare īn fosa glandei lacrimale cu extindere īn portiunea superolaterala a orbitei posterioare si fosa temporala. S-a intervenit chirurgical prin abord superolateral si s-a ablat īn totalitate tumo-ra urmata de reconstructia plasica a regiunii laterale a pleoapei superioare. Functia vizuala este recuperata complet si aspectul estetic bun. Evolutie foarte buna fara recidive īn ultimii doi ani.

English - Abstract
The authors present the case of a 63 years old patient, admitted in the Neurosurgical Departament, presenting exulcerated adenoid cystic carcinoma of the right lacrimal gland. The tumor was extended medially to the superolateral part of the posterior orbit and laterally in the temporal fossa. The patient presents one and a half years history of left exophthalmos, the tumor exulcerated in the left temporal region one year ago, mean-time the tumor extended in the superior half of the posterior orbit. The tumor was completely removed using superolateral orbital approach, with very good postoperative course. Complete recovery of the visual function and satisfactory aes-thetic aspect was obtained, without the risks of a craniotomy. Good outcome without recur-rence in the past two years
